A sharp, lower back pain, usually on one side—sometimes stabbing when you stand up, other times a dull ache after sitting for too long: This is often caused by the sacroiliac (SI) joint, the strong connection between your spine and pelvis. Many people come to us saying, "My SI joint is blocked," a phrase they either heard from a doctor or found on Google. Together, we'll uncover what's really going on, and the great news is: it's highly treatable.
What the SI Joint Is and Why It Hurts
The SI joint barely moves, yet it carries a massive load: with every single step, it transfers the power between your upper body and your legs. When subjected to unfamiliar strain—such as prolonged sitting, repetitive work, pregnancy, or stepping into empty space—the joint, ligaments, and surrounding muscles react with irritation. Typically, this presents as a one-sided pain just above the buttocks that pinches when rolling over in bed, standing up, or climbing stairs, and sometimes radiates into the thigh, though rarely below the knee.
"Blocked" – What Does That Actually Mean?
The concept of a "blocked" SI joint is widely known, and it perfectly describes how it feels. However, from a mechanical standpoint, the joint is rarely actually jammed. Most of the time, irritated structures and protective muscle tension create that feeling of tightness. For you, this is great news: what is caused by tension and irritation can be released—and, above all, actively influenced by you.
How We Treat It
Everything starts with a thorough assessment: using clinical tests, we pinpoint whether your SI joint is the root cause, or if your lumbar spine, hips, or musculature are playing a part. Osteopathy is truly in its element when treating the SI joint: it addresses the interplay between the pelvis, spine, and hips using hands-on manual techniques to release the surrounding protective tension. Physiotherapy then secures and stabilizes your progress: targeted exercises for your core, glutes, and hips restore the control and support your pelvis needs. Both therapies work hand in hand, following one unified plan.
The SI Joint and Pregnancy
During pregnancy, hormones naturally loosen the ligaments around the pelvis, causing the SI joint to lose some of its stability and often make itself felt. Gentle, targeted treatment and customized exercises can provide noticeable relief; learn more on our Pregnancy Page.

When to take charge and see a doctor
Back pain is rarely dangerous. However, seek immediate medical evaluation if you experience any of these signs:
Paralysis or sudden weakness in your foot or leg
Numbness in the genital area
Loss of bladder or bowel control
Fever in addition to the back pain
Pain after a fall (for older adults, even a minor one counts)
Deep lower back pain that wakes you up at night or early in the morning for weeks, accompanied by stiffness (this could indicate an inflammatory cause)
These symptoms are rare. If none of these apply to you, nothing stands in the way of your successful treatment with us.
Who covers the cost? Let's talk investment in your future.
